Patients are seen within 5 minutes of their scheduled appointment over 95% of the time so please be on time and try to arrive early!

Be sure to bring the following:

• Your insurance card(s) & Photo ID

• Physician referral forms if required by insurance

• A list of current prescription or over-the-counter medications you are taking, including dose and frequency

• Pertinent information about your medical and surgical history, including prior medical records

• Any recent x-rays or relevant test results

• Assistive devices such as canes, walkers, glasses or hearing aides

• Completed patient forms.

• Comfortable clothing & shoes
